Tech4Impact Diffusion Grants Program Proposals Received

During this last month we have been pleased to see the rollout of the Administration on Aging (AoA) and Centers for Medicare and Medicaid Services (CMS) Aging and Disability Resource Center (ADRC) grants to 16 states. It is particularly important to note that many of these programs, in addition to improving access to care, are expanding the use of evidence-based care transition programs for older adults and persons with disabilities. These care transitions models have demonstrated they can lead to a reduction in avoidable hospitalizations, improvements in health outcomes and cost of care, and an increase in the number of people that are able to safely and effectively transition from hospital to home or to other community settings.

The Center for Technology and Aging is pleased to support these initiatives through its Tech4Impact Diffusion Grants Program. The Tech4Impact Diffusion Grants Program will provide up to six one-year grants to organizations successfully proposing programs that will assist in accelerating of adoption of technologies that enhance care transition strategies by Aging and Disability Resource Centers. Once again, the Center for Technology and Aging would like to commend the Administration on Aging (AoA) and Centers for Medicare & Medicaid Services (CMS) for their leadership in advancing the ADRC program as a means of improving access to care. CTA will be sharing the progress of ADRCs as they implement technology-enhanced care transition programs in 2011 and, as always, will welcome suggestions and input from the field.
