$500,000 Awarded in Post Acute Care Transitions Technology Diffusion Grants

Technology has significant potential to reduce inappropriate health care utilization at a critical juncture in the health care delivery process: post acute care transitions (PACT). CTA is honored to have been invited to collaborate with the Administration on Aging (AoA) and Centers for Medicare & Medicaid Services (CMS) in their effort to expand evidence-based care transitions nationwide through Aging and Disability Resource Centers (ADRCs).

Our participation is through a grant program called “Technologies to Improve Post Acute Care Transitions,” or Tech4Impact. We designed it specifically to complement and supplement the AoA/CMS ADRC $68 million Evidence-Based Care Transition Program initiative.

We have awarded grants to five outstanding state units on aging – California, Indiana, Rhode Island, Texas and Washington. They have proposed using technology-enhanced transitional care interventions to rapidly reducing avoidable hospitalizations, improve health outcomes, and increase the number of people that are able to safely and effectively transition from hospital to home or similar settings.

These Tech4Impact grantee programs are expected to reduce the costs of care while improving the quality of care for older adults and persons with disabilities.

Two grantees are expanding use of technologies that enhance care transitions program evaluation and planning, and three grantees are expanding use of technologies that better empower patients/consumers in the care transitions process.

CTA will disseminate the grantees’ program learnings, including tools, recommendations and diffusion strategies throughout the January 2011 to December 31, 2011 grant cycle and beyond.
